Archives & Student Records
Below are procedures for requesting copies of student record information. The Archives Department retains records for students who have withdrawn from our District and students who graduated from the District more than one year ago. If you are a recent graduate (graduated during the last school year), please contact the high school you last attended for copies of your student records.
The Archives Department is open from 7:30 a.m. until 2:00 p.m., Monday-Friday. To make arrangements to examine your record or request copies in person, please call 360-662-9270. You can fax your request to 360-662-9271. Please follow the instructions listed below.

Transcript or Records
Procedures for Requesting/Releasing Information from Student Records
The Archives Department retains records for students who have withdrawn from our District and students who graduated from the District more than one year ago. If you are a recent graduate (graduated during the last school year), please contact the high school you last attended for copies of your student records.
All requests must be in writing and must include all information below:
- Student's full name (no nicknames)
- Birth date
- Last CKSD school attended
- Year graduated or withdrawn
- Items to be copied
- Complete name and address of where to send copies
- Purpose of disclosure

- If student is under eighteen years of age, the parent/guardian must sign.
- If the student is over eighteen years of age, the student must sign.
- If the student is over eighteen years of age, but is claimed as a dependent for income tax purposes (see Excerpts from the Internal Revenue Code); either the student or parent may sign
If you are another educational institution, you may send a written request on your school letterhead.
Please provide the required information on an official CKSD records request form or on a separate sheet of paper. Requests can be faxed to the Archives Department at 360-662-9271, or mailed to:

Verify Graduaton Date
Graduation Verification for Employment Purposes
- If the former student graduated in the last school year, contact the last CKSD school they attended.
- If the former student graduated more than one year ago, contact the Archives Department. To obtain verbal verification, call 360-662-9270. To obtain written verification, fax your request to 360-662-9271, or mail the request to: Archives Department, Central Kitsap School District, PO Box 8, Silverdale, WA 98383-0008. Your request must contain the following information:
- Name of Employer
- Telephone number or email address where you can be reached
- Full name while in school (no nicknames) and birth date of former student
- School and graduation date to be verified
- Release of Information signed by the former student

Special Services Department
If you need to request information from your special services records, please contact the Special Services Department between 8:00 a.m. and 4:30 p.m. at 360-662-1040. You may also fax your written request to 360-662-1041.
If you are another educational institution, you may send a written request on your school letterhead.
Family Education Rights & Privacy Act of 1974
Excerpts from the Family Education Rights and Privacy Act of 1974
- 99.5 What are the rights of students?
- (a) When a student becomes an eligible student, the rights according to, and consent required of, parents under this part transfer from the parents to the student.
"Eligible Student" means a student who has reached 18 years of age or is attending an institution of post-secondary education. [Authority: 20 USC § 1232g(d).]
- 99.30 Under what conditions is prior consent required to disclose information?
- (a) The parent or eligible student shall provide a signed and dated written consent before an educational agency or institution discloses personally identifiable information from the student's education records, except as provided in 99.31.
- (b) The written consent must:
- Specify the records that may be disclosed;
- State the purpose of the disclosure; and
- Identify the party or class of parties to whom the disclosure may be made.
- 99.31 Under what conditions is prior consent not required to disclose information?
- (8) The disclosure is to parents of a dependent student, as defined in the Internal Revenue Code, 26 USC§152.
